Some users are having issues regarding the upgrade scripts, looks like it works in general, but sometimes they are not triggered.

There's some discussion regarding the upgrade of plugins (pkp/pkp-lib#7987), for now the plugin can ensure that upgrades can be re-executed and run them all the time (or behind a "flag").
